New Ohio Department of Health Immunization Requirements
The Ohio Department of Health (ODH) has made changes for Ohio school children in regards to immunization requirements starting with the 2016-2017 school year.
All 7th graders will be required by the State of Ohio to have proof of the Meningococcal (MCV4) vaccine and TDaP on file BEFORE the start of 7th grade.
All 12th graders will be required by the State of Ohio to have proof of the Meningococcal (MCV4) vaccine on file BEFORE the start of 12th grade.
The meningococcal vaccine must be given after the age of 16 years old. If your child had a dose of MCV4 before the age of 16, they are required to have a 2nd dose.
Doctor appointments will fill up fast, so please do not wait until August. This may impact your child's ability to start school on time.
You may bring your child's updated immunization form to the appropriate school office, to the Board of Education Office or fax to 740-355-8975 (Attention: School Nurse).
